Model Withdrawal Form

Last updated: 15 July 2026 — Effective date: 15 July 2026 — Legal basis: Directive 2011/83/EU; Czech Civil Code §§1829, 1832

App Store purchases: If you bought your subscription inside the iOS app, Apple is the seller of record. Cancellations, refunds and the 14-day right of withdrawal for App Store purchases are handled by Apple — request them at reportaproblem.apple.com, or cancel via Settings → [your name] → Subscriptions on your device. Neither we nor Polar can issue refunds for App Store purchases. This form applies only to web purchases made at ohmylore.app, which are processed by Polar as our Merchant of Record.

1. About This Form

This is the standard EU model withdrawal form for consumers exercising their right of withdrawal from a distance contract, as provided for by Directive 2011/83/EU on Consumer Rights and implemented in Czech law under Czech Civil Code §1829.

Before completing this form, please read Section 2 (Important Notice on Waiver) below. In some cases, by requesting immediate access at checkout — through the express-consent checkbox shown before payment — and receiving the service, you may already have lost the right of withdrawal under Czech Civil Code §1837(l).

2. Important Notice on Waiver (§1837(l))

Before you check out on the web at ohmylore.app, you are shown an unticked checkbox that reads, in substance: “I request immediate access to Oh My Lore and acknowledge that I thereby lose my 14-day right of withdrawal once the service has been fully performed; for ongoing subscriptions I acknowledge my withdrawal right is limited as described in the Refund Policy.” If you tick this box, thereby giving your express prior consent, and we begin providing the digital service, you lose the right of withdrawal for that purchase in accordance with §1837(l) of the Czech Civil Code and Article 16(m) of Directive 2011/83/EU. We keep a dated record of this consent (text and version included), confirm it to you by email on a durable medium after your purchase, and make a copy available to you on request. Submission of this form in such circumstances does not entitle you to a refund.

If this consent was not captured — for example the checkbox was not presented, or you did not tick it — the statutory 14-day right of withdrawal applies in full and you may use this form.

3. Send Your Withdrawal Notice

To exercise the right of withdrawal, complete and send this form (or any other unequivocal statement of your decision to withdraw from the contract) to:

Email support@ohmylore.app
Subject: “Withdrawal”

You can also send a free-form email containing the information below. This form exists for your convenience; no specific format is legally required.

4. Withdrawal Declaration Form

Complete and submit the information below by email to support@ohmylore.app with the subject “Withdrawal”:

I/We (*) hereby give notice that I/We (*) withdraw from my/our (*) contract for the provision of the following digital service:

e.g. “Oh My Lore! Rare plan — monthly” or “Oh My Lore! Legendary plan — annual”

(*) Delete as appropriate.

5. Refund Mechanics

The refund is issued through Polar Software, Inc., our Merchant of Record, to your original payment method within 14 days of your valid withdrawal notice (Czech Civil Code §1832). We are not permitted to charge you for the refund. Polar issues the refund on our behalf because Polar is the seller of record and collected your payment.

When AI-credit top-ups are offered: if you have used any AI Credits purchased as part of your order, those used credits will have lost the right of withdrawal under §1837(l) and will not be refunded.

6. After You Submit

Once we receive your valid withdrawal notice, we will:

  1. Acknowledge receipt by email within a reasonable time.
  2. Verify your eligibility (that the right of withdrawal has not been waived).
  3. Initiate the refund through Polar Software, Inc. within 14 days of receiving your notice.
  4. Terminate your access to the relevant paid features on the date of withdrawal or refund (whichever is earlier), unless you have a separate active subscription.
